Municipal public expenditure: An ecological study of resource allocation and health outcomes

Public health systems face the problem of underfunding and inefficient resource utilization, particularly in resource-limited countries like Brazil. This research seeks to identify specific patterns of resource allocation that correlate with improvements in health indicators in medium-sized municipalities in Brazil between 2010 and 2019. The research is characterized as an exploratory ecological study, using a spatiotemporal approach to examine data over the proposed period. The analysis involved classifying municipalities based on public spending and health indicators, using descriptive statistics, clustering techniques, and Bayesian networks to identify patterns and trends. The Bayesian networks identified dependency patterns between public spending variables and health indicators, revealing that increased investment in primary care is associated with improved vaccination coverage and a reduction in hospitalizations for preventable conditions. Additionally, the models showed that resource allocation in health influences service efficiency, mitigating adverse socioeconomic factors in more vulnerable regions. This study provides insights for health managers and policymakers, highlighting the need for allocation strategies that prioritize both efficiency and equity. The conclusions also pave the way for future research that integrates the social determinants of health into public policy analyses.
